Family Library Card Application
By Ohio law, your library records are confidential, and the Wadsworth Public Library will no longer allow spouses or other family members to use each other's library cards. Instead, we offer a Family Library Card which allows all members of a family to consolidate their borrowing records into a single account.
If any members of your family have personal library cards, those cards must be returned when picking up your new Family Card. All outstanding charges and fines on all cards must be cleared at that time.
PLEASE READ BEFORE SUBMITTING
- This form will register you as a borrower of material from Wadsworth Public Library.
- Applications will be processed within three working days, and your card will be available at the Wadsworth Public Library check-out desk.
- You will be asked to present one I.D. with your current address when you pick up your family card. (driver's license, checkbook, employment identification, school identification, or envelope or postcard with a recent postmark received at the address given, or utility bill)
As library card holders, we will:
- Observe the rules of the Library.
- Be financially responsible for all charges on any late, damaged, lost, or stolen materials borrowed on this card.
- Report the loss or theft of this card immediately to avoid unauthorized use.
- Update my name, address, and phone number whenever necessary.
